Short description and discussion of imaging findings:
The 26-year-old female was referred to MRI for evaluation of an adnexal mass. She had a history of resection of a left-sided ovarian teratoma. The MRI shows a multilocular cystic pelvic mass originating from the right adnexal region with different signal intensities in different cyst compartments and presence of complex non-cystic components. Despite lack of evidence of lipid-containing components an ovarian teratoma was assumed. Pathological work-up after resection proved struma ovarii, which is a monodermal mature ovarian teratoma subtype composed predominantly (at least 50%) of thyroid tissue. Thyroid follicles with variable amounts of colloid material result in variable size and signal intensities of cystic compartments in MRI. Struma ovarii accounts for approximately 3% of mature ovarian teratomas.
